Inpatient Care Management Nurse RN - Remote id-19076

At CVS Health, we are looking for a dedicated Inpatient Care Management Nurse to provide high-quality care coordination for our members. In this role, you will support patients through complex healthcare needs, ensuring they receive the best possible care while managing costs effectively.

Responsibilities

  • Conduct clinical assessments of inpatient admissions to ensure medical necessity and appropriate levels of care.
  • Collaborate with healthcare providers, patients, and their families to create and implement individualized care plans.
  • Monitor patient progress, coordinate discharge planning, and provide education on post-discharge care.
  • Ensure compliance with medical policies, guidelines, and regulatory requirements.
  • Utilize clinical expertise to review medical records, determine treatment appropriateness, and authorize necessary services.
  • Work with a multidisciplinary team to optimize patient outcomes and resource utilization.
  • Document all interactions and decisions in accordance with company policies and HIPAA regulations.
