This full-time remote position offers an exciting opportunity to make a meaningful impact in the lives of our patients while working from the comfort of your own home. As an RN Case Manager, you will be responsible for providing care coordination, patient education, and advocacy to help improve outcomes and ensure a positive healthcare experience.

Responsibilities:

  • Coordinate care and manage a caseload of patients with various healthcare needs, ensuring continuity of care and best outcomes.
  • Conduct comprehensive assessments and develop individualized care plans based on patient needs.
  • Monitor patient progress and adjust care plans as necessary to ensure timely and appropriate interventions.
  • Collaborate with physicians, nurses, and other healthcare professionals to deliver comprehensive care.
  • Educate patients and families on managing their conditions, treatments, and available resources.
  • Ensure compliance with healthcare regulations, organizational policies, and clinical standards.
  • Advocate for patients by identifying and addressing barriers to care and providing support in navigating the healthcare system.
  • Document patient interactions and maintain accurate records in accordance with legal and organizational requirements.

Requirements:

  • Active Registered Nurse (RN) license in the state of practice.
  • Minimum of 2 years of clinical experience in a hospital, home health, or case management setting.
  • Strong communication skills, both written and verbal.
  • Ability to work independently, manage time effectively, and prioritize tasks.
  • Experience with electronic medical records (EMR) systems.
  • Ability to problem-solve and make decisions in a fast-paced environment.
  • Previous case management or care coordination experience is preferred but not required.
  • BSN (Bachelor of Science in Nursing) is preferred but not required.
  • Certification in case management (CCM) is a plus.
